Hard Disk issue in server

Hi,
I have RHEL 4 dedicated server - i that server i face filesystem issue - the issue is that the filesystem is changed to read-only mode and services ( apache,mysql ) are stopping - when in try to edit any file or change the ownership of file it shows the error like :
chown: changing ownership of `about.php': Read-only file system
I have ran fsck and e2fsck but no use
What can i do plz help me to get out of this issue

Did you run fsck under single user mode?

There is a reason why the OS puts FS into read-only mode. You may have a bad disk.
Can you check your FS setting for "errors=remount-ro" in /etc/fstab?

Yes in ran fsck and e2fsck in signel usermode...

There is no such entries "errors=remount-ro" in my fstab

That's odd. Have you looked at dmesg/syslog and see what caused the OS to put FS into read-only mode? Things don't happen for no reason. And, it's very likely logged.

i can see the line like the following in dmesg
EXT3-fs warning: maximal mount count reached, running e2fsck is recommended
EXT3 FS on dm-0, internal journal
EXT3-fs: mounted filesystem with ordered data mode.
Adding 2048276k swap on /dev/sdb2. Priority:-1 extents:1
Adding 1959920k swap on /dev/sda3. Priority:-2 extents:1
Also is there any possiblity for hacking ? and keep my FS read only - if so how do detect those ?
